Why is broadband more expensive in the US

Not only is the question: Why it is more expensive, but why isn't it readily available in all areas of the country? Such as rural areas, or inner city, or lower income areas?
A large majority of Americans are either on extremely slow DSL, Dial-Up, or no internet at all.
Some have to resort to traveling to public places that offer WiFi, or access to public computers, such as Public Libraries.
